To have documents prepared, notarized, notarial true copies of documents made, affidavits or declarations prepared or sworn, you should bring with you two pieces of personal identification, one of which should bear your photograph (for example, Ontario driver's licence or health insurance card, passport, or other identification).
If you wish to have documents notarized or notarial true copies of documents made, you should also bring the original documents.
If you have not known an eligible guarantor for at least two years, attend at any Passport Canada office, obtain a "Statutory Declaration in Lieu of Guarantor" form, complete it, and attend at my office to have it notarized. You should also bring the originals of any documents referred to in the form.
